Follow these steps to order your new prescription. If you're ordering
refills or refills as well as new prescriptions, go to Order
a Refill Online.
- Go to our Prescription
Products page
and search for the medication(s) you require. Choose the correct
dosage and quantity, and click "Add
to Cart".
- When you've added all the medications
you need to your cart, click "Proceed to Checkout".
- Read
the Customer Agreement, and click on "Proceed to
Billing".
You will be asked to sign in to your account. If you are a new
customer, fill out the "First-time customers" section.
- Complete
and submit the Checkout Process.
- Fax your prescription(s) to MagenDavidMeds.com
at 1-877-232-8824 (toll free.)
New Order by Fax
Follow these steps to order your new prescription. If you're ordering
refills or refills as well as new prescriptions, go to Order
a Refill by Fax.
- Go to our Downloadable
Forms page.
Download and print a "New
Order Form." If you're a new customer at MagenDavidMeds.com,
complete the full form and sign the Customer Agreement. If you've
already completed a Customer Agreement at our pharmacy, you do
not need to resubmit it.
- Complete the Order Form, printing clearly.
- Download
and print a "Fax Coversheet" and a "Customs
Declaration" to complete and submit along with your order.
- Fax
your forms, along with valid prescription(s) for all drugs ordered,
to 1-877-232-8824 (toll free.)
